Elpat's Program

The primary role of a home care nurse is to oversee the patient's health in collaboration with their physician. The goal is to help patients live fulfilling, healthy lives and achieve independence at home. Here's an overview of what to expect during visits from an Elpat nurse.

A nurse will call you to schedule a home care appointment within 48 hours of a hospital or nursing facility discharge. The nurse will provide necessary services during the first visit, such as:

• Describe the services and secure written permission to provide home care
• Review your medications
• Assess for fall risk
• Perform a safety assessment
• Obtain your vital signs
• Assess your pain
• Discover what your health goals are

The nurse will collaborate with you to develop a customized plan to assist you in reaching your goals, such as wound care, medication management, diabetes management or injections. The initial visit usually lasts a few hours, depending on the patient's needs.

Subsequent nursing visits after the admission visit are specifically tailored to empower you to attain your health objectives. These vital nursing visits typically encompass the following:

• Vital signs
• Wound treatment
• Medication teaching
• Disease monitoring
• Diabetes Management
• Infusions
• Injections
• Pain management
• Disease teaching
• Infection Prevention

Home care nursing visits vary based on the patient's needs, with some requiring daily visits and others just once a month. Visits are more frequent initially and taper off nearing discharge, aiming to teach patients to live independently and in better health.
